It's from the Star Wars Chronicles mod. It hasn't been updated for a while, I don't know if it's still in

development. The skin hasn't been released.

 

But there's an Emperor skin you might like. It doesn't look as good as the one Darth Phae made,

but it's at least an alternative: http://jkhub.org/files/file/747-darth-sidious/
