Have you tried Noesis? It can export to many formats, including MD3.

From what I can tell, Noesis cannot import the model so it's out. Blender I can only find exporters. As Psyk0sith said, would be best to ask CoD modders because the first step is to import the model.

I'd say give COD4 Mod tools a go. I've downloaded it once, and I believe it had an xmodel importer (xmodel is the format of COD4 and other COD's as I recall) aswell for other series of that game.

but I suggest to not use COD4 models. Use Counterstrike/Half Life/Valve engine models instead
