Notepad and dry erase board. I put some ideas down on notepad, and if I need to expand on them draw them out on the board.

You don't really need to plan a whole lot. I have a notepad that I use for very complex math and helping me visualize it a bit sometimes, but not usually.

Usually I just think about how I would do everything from a functional point of view.
