5. A statement, under penalty of perjury, that the information provided in your notice is accurate and that you are the copyright owner or authorized to act on behalf of the copyright owner.
6. Your physical or electronic signature.
Counter-Notification
If you believe that material you posted was removed or disabled by mistake or misidentification, you may submit a counter-notification. Your counter-notification must include:1. Identification of the material that has been removed or disabled and the location where it appeared before removal.
2. A statement, under penalty of perjury, that you have a good-faith belief that the material was removed or disabled as a result of a mistake or misidentification.
3. Your contact information, including your full name, mailing address, telephone number, and email address.
4. A statement that you consent to the jurisdiction of the federal court in your judicial district (or the district where JKHub is located) and that you will accept service of process from the person who filed the original DMCA notice or their agent.
5. Your physical or electronic signature.
